Blue Skies transformation service is designed to live out its mission to improve the health of the communities we serve. We serve commit our skills and resources to the benefit of the whole person by providing the finest in health care, while addressing the physical, emotional, and spiritual needs of individuals and their families. We further pledge to support the research and education efforts for our partner, the National Hospice and Palliative Care Organization.

Our staff brings the most advanced technology, research, and expertise available to treat illness. Blue Skies offers a very specialized team to work with your physician to help relieve physical, emotional, and spiritual suffering, while helping make the most of life for people with life threatening illnesses, and their families. The team addresses physical, psychological, social, and spiritual needs, and can help patients cope with the pain and anxiety that come with major health changes.

The Transformation and Life Choices (TLC) team provides coordinated care for people with life threatening illnesses and their families. Team members include:

A doctor who specializes in recommending medications or treatments to help relieve pain and symptoms caused by illness or aggressive curative treatment.

A nurse with advanced training in life threatening illness and coordinating care in the hospital, the patient’s home or in skilled care facilities.

A social worker with advanced training in counseling patients and family members about interpersonal issues, lifestyle changes, hospital discharge planning and coordination of community resources.

A chaplain who offers spiritual and emotional support, counseling, and coping strategies for patients and families.

Volunteers that are devoted and committed to patient care.

The TLC team offers special expertise in addressing issues surrounding life threatening illness. Working together to support the patient and family, clearly communicating with existing providers about interpersonal issues, lifestyle changes, hospital discharge planning and coordination of community resources.

The TLC team incorporates faith and individual needs into treatment, care, and counseling.
